Colorado is a direct-access state, which means licensed physical therapists are legally allowed to evaluate and treat patients without a referral from a physician. You can pick up the phone, call a clinic, and book your first appointment without seeing a doctor first.

Why direct access matters

Before direct access laws, patients with back pain or a sore shoulder typically had to schedule a doctor's visit first, get evaluated, and obtain a referral before they could see a physical therapist. That added weeks, sometimes months, to recovery — and in many cases, the doctor's visit just resulted in being told to "try PT" anyway.

Research consistently shows that patients who see a physical therapist early heal faster, need fewer medications, and are less likely to need imaging or surgery. Direct access removes the gatekeeper that was slowing recovery down.

When you might still want to see a doctor first

Direct access isn't the right path for every situation. You should see a physician first if:

  • You've had recent trauma or fall and might have a fracture
  • You have new neurological symptoms (significant weakness, loss of bladder/bowel control)
  • You have unexplained weight loss, fevers, or systemic symptoms with your pain
  • Your symptoms feel sudden or severe and don't seem musculoskeletal

Beyond those red flags, the vast majority of musculoskeletal complaints — back pain, neck pain, joint pain, post-surgical recovery, sports injuries, chronic pain — are appropriate for direct PT access.

What about my insurance?

This is the one important caveat: while Colorado state law allows direct access, your specific insurance plan may still require a referral for coverage. Most plans don't, but some do.
