When you schedule your first appointment, we'll email you the relevant intake forms to complete before your visit. This saves time at check-in and lets us focus on your evaluation.
How patient forms work.
After you schedule your first appointment, we will email you the intake paperwork to complete at home. This typically includes:
- New Patient Intake (medical history, current symptoms, goals)
- Insurance Information
- HIPAA Notice of Privacy Practices acknowledgment
- Consent for Treatment
- Financial Responsibility Agreement
- Outcome Measure questionnaires specific to your condition (e.g., back pain index, shoulder function scale)
If you'd prefer to fill them out at the clinic, just arrive 15 minutes before your appointment and we'll have everything ready for you.
